After scheduling your appointment, you will receive an email with a link to a structured intake form.
The intake form gathers information about your long-term bodily patterns and your current health challenges to provide context for the session. This helps in determining your normal baseline state and what may be affecting it right now.
Completing the form in advance ensures our session time is focused and productive
What to expect
- Questions about how your body has felt over the years
- Your current symptoms, energy, sleep, digestion, and daily functioning
- Any recent changes in routine, diet, stress, or supplements
Time estimate
Approximately 10–15 minutes to complete the form.
Note: I do not prescribe medications, diagnose medical conditions, or provide guidance on starting, stopping, or adjusting any prescription or over-the-counter medications.

Please be prepared to list any supplements, herbs, and medications you are currently taking to help provide context for your intake.
